What happens when AI uses an outdated company document?

Why document updates and version control matter when employees start using AI.

An AI answer can accurately summarise a document and still give the wrong guidance for today. The problem is not always the summary. Sometimes it is the version underneath it.

Yesterday’s rule can sound current

Consider a purchasing procedure that changes its approval threshold. An older copy remains in a shared folder while the approved version sits elsewhere. If the older copy is retrieved, the answer may present yesterday’s process without any obvious warning.

The same issue can appear in onboarding guides, customer terms, escalation procedures and internal technical instructions. Making documents easier to query does not automatically make every document current.

A newer file is not always the approved file

Modified dates tell only part of the story. A recently edited draft may not be effective yet. An older approved procedure may still be the correct authority. Teams need a way to distinguish drafts, approved versions and superseded material.

Keep ownership, approval status and effective dates clear at the source. Decide how historical versions should be handled: retained for reference, excluded from routine answers, or surfaced only when someone explicitly asks about the past.

Updates must reach the answer layer

Ask what happens after a source changes. How does the system discover the update? When is the new content available for answers? What happens to deleted documents and cached results? A connection to a repository is not, by itself, proof that every change is immediately reflected.

Test this with a non-sensitive sample document. Ask a question, update the approved answer in the source, and repeat the question after the expected refresh interval. Check the citation and version as well as the response text.

Give conflicting information an owner

When two sources disagree, someone needs to resolve the underlying content. An AI interface should not become the place where conflicting policies are silently reconciled.

Start with a small set of well-owned documents. Agree who reviews problems and how corrections are made. Expanding from a dependable knowledge set is easier than trying to clean up every document after people begin relying on it.

The takeaway

Current answers depend on clear source ownership, version handling and a tested update process—not simply connecting more files.
